To qualify for your rebate, purchase any of the PocketDISH products listed above between 11/15/05 and 4/15/06. And , if you're not already, become a DISH Network subscriber in the same time period. Finally, follow these steps to receive your rebate:


1. Download and fill out the PocketDISH Rebate Form completely
2. Include your dated sales receipt or invoice and CIRCLE the qualifying purchase.
3. Include the original UPC code from the PocketDISH packaging.
4. Include a copy of your current DISH Network statement dated 11/15/05 or later.
